The Montefiore ACO was one of the original Pioneer Model ACOs, and achieved savings for the Medicare program in each of the 5 years of the program. Medicare benefited by over $78 million over five years, with quality scores that exceeded 95%. Montefiore is now responsible for managing the care, quality and outcomes of nearly 50,000 Medicare beneficiaries attributed to its Next Generation ACO.

Montefiore Health System currently manages over 400,000 lives through various value-based arrangements, which include over 210,000 full risk capitated lives with several health plans covering Medicare, Medicaid and commercial product lines.
